HepLink Care Coordinator

STATUS: Full time/Non-Exempt 

POSITION SUMMARY 

The HepLink Care Coordinators link people living with Hepatitis C to treatment and care.  Case management/linkage to care services may be offered at the Step-Up office, or in the field, at shelters, encampments or in the client’s home. The position may require the ability to travel from the office to these locations.

HepLink Program Manager 

ESSENTIAL DUTIES AND RESPONSIBILITIES 

Other duties may be assigned.  

Linkage to Care 

  • Respond to referrals to establish care with clients and assess needs and strengths. 
  • Develop a plan with clients to begin and complete treatment for Hepatitis C. 
  • Provide education, support, facilitation and referrals until the client has achieved a sustained virologic response (SVR) against Hepatitis C. 
  • Facilitate transition of care to other longer-term programs if needed. 

Outreach 

  • Engage clients in the field at shelters, treatment centers, and encampments. 
  • Provide occasional point of care testing for Hep C. 
  • Collaborate with community partners to improve access to Hep C treatment.  

Program Administration 

  • Complete documentation of client care accurately and in a timely manner. 
  • Submit administrative and program reports on or before deadlines. 
  • Participate in program and agency meetings and training. 

COMPETENCIES

  • Proficiency with Word and Outlook, ability to learn additional programs required. 
  • Collaboration with clients in ways that communicate clearly and respectfully. Ensure that interactions are client-centered and avoid re-traumatization.

This position does not supervisor others.  

Experience and Education – One or more of the following: 

  • Work Experience: One or more years working as a community health worker, case manager, care coordinator, or linkage to care worker. 
  • Lived Experience: Living with, in recovery from, or caring for others with, viral hepatitis, HIV, substance use disorder, homelessness, trauma, mental illness, justice involvement, etc. or has attempted to access healthcare as a member of a community that has been affected by social determinants of health. 
  • Certification: Certification as a Community Health Worker Certification, Peer Recovery Coach Certification, or Indiana Addictions Professional, or equivalent. 
  • Education: Associate’s degree in sociology, psychology, counseling, pastoral counseling, family and child development or related field

Language:  

  • Read and speak English required.  (Conversational fluency in other verbal and sign languages are valued.) 

Physical: 

  • The ability to consistently work 37.5 hours per week. Additional hours may occasionally be required.  
  • Vision abilities required by this job include close vision, distance vision, peripheral vision and ability to adjust focus 
  • While performing the duties of this job, the employee is regularly required to sit, stand, walk, reach with hands and arms, use hands to finger, handle, or feel, and talk or hear. 
  • Ability to bend, twist and squat. 
  • The use of a computer, phone, copiers and other office machines.  
  • The occasional lifting of objects weighing up to 25 pounds. 
  • Verbal communication with people who speak English.  
  • Other: Dependable, independent transportation to and from the office and to meet clients in the field.

Pay: $42,500.00 – $44,500.00 per year
