Non-Medical Case Manager

STATUS

Full time/Non-Exempt

POSITION SUMMARY

The Non-Medical Case Manager provides case management services to individuals living with HIV in Indiana.

REPORTS TO

Senior Program Manager

ESSENTIAL DUTIES AND RESPONSIBILITIES

Other duties may be assigned.

  1. Provide individualized case management to persons living with HIV in Indiana.
  2. Link clients to medical care and other services such as insurance programs, housing, nutrition, medication assistance, etc.
  3. Work effectively with diverse populations.
  4. Maintain high level of confidentiality and comply with HIPAA regulations.
  5. Enter and maintain complete client data and case notes in a timely manner.
  6. Attend all trainings and meetings as required by Step-Up and/or the appropriate agency.
  7. Work independently after training is completed.
  8. Provide services at various locations throughout Central Indiana
  9. Perform general administrative duties as assigned, including copying, mailing, ordering supplies, answering phones, etc.

Education/Experience:

Bachelor’s degree preferred. Equivalent experience will be considered.

2+ years experience in case management preferred.

A combination of lived expertise, work experience, and/or certification as defined below may be considered in lieu of degree.

Lived Expertise

Living with, in recovery from, or caring for others experiencing, viral hepatitis, HIV, substance use disorder, homelessness, trauma, mental illness, etc.

Experience accessing healthcare as a member of communities that have been traditionally excluded from access including Black, Indigenous, and other People of Color (BIPOC); lesbian, gay, bisexual, transgender, queer/questioning, intersex (LGBTQIA+); physical ability, or national origin, people who have experienced homelessness, or anyone with a history of criminal justice involvement.

Language Ability:

Ability to read and speak English required.

Other Skills:

Proficiency with Word, PowerPoint Excel, and Outlook, and ability to learn additional programs, required.

Dependable transportation required; position will entail weekly travel in Central Indiana, as well as travel throughout the state of Indiana. In some instances, overnight stays may be necessary. Out of state travel with overnight stays may be required for conferences, training, and continuing education opportunities.

Supervisory Responsibilities:

This position does not directly supervise.

This job is performed primarily in an office setting; While performing the duties of this job, the employee is routinely required to use standard office equipment such as computers, phone, photocopiers, filing cabinets and fax machines, and is occasionally exposed to moving mechanical parts; The noise level in the work environment is usually low to moderate.

The employee must regularly work 37 ½ hours per week, additional hours may occasionally be required.

While performing the duties of this job, the employee is regularly required to sit, reach with hands and arms, talk, and hear. The employee frequently is required to use hands to touch and handle objects. The employee frequently stands or walks.

The employee must occasionally lift and/or move up to 25 pounds.  Specific vision abilities required by this job include close vision and the ability to adjust focus.
