Connections Plus Community Navigator

The Connections Plus Community Navigator provides education and resources to people who are at risk for HIV/HCV and STDs through referral to Step-Up’s PrEP program or other appropriate prevention resources. The navigator will address unmet need(s) reported by a client through referring the client to in-house and external resources, including food, housing, transportation, and other social determinants of health resources.
